Prepare for your appointment by having the following documents ready:
- A Registration Form
- A Financial Statement
- Medical History Form

Step 3.
Send these documents by email prior to your visit.
Make contact prior to your appointment should you require additional/specific products related to your service.

Step 4.
Wait for a Bundaberg Nurse to knock on your door as they make their way to you for your scheduled service.

Intended day: Saturday
Times between 9am and 3pm.
