Our nurses can perform the following assessments based on your needs:
- Cognitive Impairment Scale (also known as a PAS)
- RUDAS
- Mini-Mental State Exam
- Dementia Scale
- Behavioural Assessment
Bundaberg Nurse offers convenience by providing a qualified registered nurse to your home to attend to cognitive and physical assessments that help to recognise and relay these changes to your GP.
The wellness assessments we use can help to reflect changes in behaviours and abailities. These assessments can be performed annually or as often as necessary.
Our nurses work with you, your family and other allied health professionals to perform and release assessments that may reflect changing needs and abilities and provide you time to plan for the future.
Home Care Package Supplements
Dementia and Cognition Supplement
If your on a home care package then you may be eligible for a financial supplement offered to people diagnosed with Dementia or those with a specific score on the above assessments.
Our nurses are qualified to perform these assessments and can submit the supplement application on your behalf.
